• Must reside in the United States
• Must be known to the administration and/or provide references
• Must provide documentation of need as requested. You will be required to fax or mail proof of the following:

    ~Identity (drivers license or government issued id card)

    ~Address (lease or utility bill or phone bill or letter from a state or governmental agency). Must provide two (2) verifications of residence.

    ~Verification of need: For example if you are requesting help on a gas bill, you must send the most current bill (not past 30 days old) You cannot use this same bill for your address verification.If you have a medical condition, you must provide proof from a physician. If it is a rental request, lease or eviction notice. And these are a few examples.

    ~Depending on request, we may require additional documentation to prove need.

• Funds to be paid directly to provider of services. 
  
• Your monetary request must be for $200.00 or less at this time (please note: Funding is not guaranteed)
